    First Week Packet Info

    NOW UPDATED (as of 9/7/17) for the 2017 - 18 school year: In an effort to streamline the process for all involved, the Hazel Wolf PTSA has created PDF fillable forms that you can complete and email to a dedicated staff email address that will receive your paperwork.

    1. FORMS THAT ARE MANDATORY FOR ALL STUDENTS
    Please save the files to your computer first (do not fill them out online in your web browser). Open and complete the forms, sign them electronically, and send as an attachment to hazelwolf.forms@seattleschools.org. You can also print out the forms, sign the hard copies, and return to the office or via Kidmail.

    2. FORMS THAT PERTAIN TO MANY, BUT NOT ALL FAMILIES
    Please look carefully through this batch and fill out the pages that apply to you. Save the files to your computer first (do not fill them out online in your web browser), open and complete the forms, sign the forms electronically, and send as an attachment to hazelwolf.forms@seattleschools.org. You can also or print out the applicable forms, sign the hard copies, and return them to the office or via Kidmail.

    3. IMPORTANT INFORMATION PERTAINING SPECIFICALLY TO HAZEL WOLF AND THE HAZEL WOLF PTSA
    Please download this file and read through this important information and optional form.

    You can also join the PTSA online, and update your school directory info at the same time. If you would prefer, you can also share your directory/contact info via our Family Registration Form. Both methods are secure and we won’t sell or share your contact information.

    4. IMPORTANT INFORMATION FOR ALL SPS FAMILIES
    Please download this file and read through this important information from Seattle Public Schools. There are no forms to fill out.

    If you would prefer to receive a hard copy of the packets home via Kidmail, please look for the form that your child(ren) will bring home with them via Kidmail the first or second week of school. You can fill it out and return it via Kidmail to receive hard copy packets.

    Please choose whichever method is most comfortable for you to complete and return this important information that we need to best serve your family.

    Other Forms

    *It's best to ALWAYS bring an ID when picking up your student from school*

    Emergency Release Form - This allows others to pick your child up from school in the event of an emergency. This comes home in the welcome packet.

    Medications at School must be filled out by the doctor for us to administer medications.

    PE Waiver Middle School ONLY - This form is needed to waive PE for a semester.
